The comfort of home.
The support to stay.
When everyday life starts to need a little more help, the right support can make home feel like home again.
Talk about your next step ↗
You may be here because…
For families who want to explore more support in a familiar place
01Daily routines are becoming harder to manage.
02A family caregiver needs a more sustainable arrangement.
03You want to understand independent caregiver referrals.
Support that fits daily life
Begin with the person: their routines, preferences, and the parts of the day that have become harder. We help you explore support for companionship, personal care, meals, and everyday activities.
A thoughtful caregiver connection
Caregiver referrals are made through Care Matters Registry, a related, distinct business connecting families with independent caregivers. Caregivers are independent contractors, not employees of Care Matters. The registry does not supervise, direct, or provide ongoing guidance to caregivers.
Room for needs to change
A change in mobility, a hospital stay, or caregiver availability may call for a different plan. We can help you reassess options and coordinate the next conversation.
How support begins
One conversation.
A more considered plan.
Listening
Tell us what’s happening.
Your concerns. Their preferences. The things that feel hardest right now. We start by listening.
Perspective
Explore your options.
Understand the possibilities with nursing insight and knowledge of the local care landscape.
Next steps
Move forward with a plan.
Bring the next steps into focus, with a clearer understanding of the support to consider.
A little more understanding
Questions,
welcomed.
You don’t need to have everything figured out before you call.
Can we ask for help before a crisis?+
Yes. A conversation can help you understand the kind of support to explore before decisions become urgent.
Are caregivers Care Matters employees?+
Care Matters Registry is a referral service for independent caregivers. The specific engagement, responsibilities, costs, and availability are discussed with you before an arrangement is made.
Is this medical home health?+
Non-medical caregiver support is different from prescribed medical home health. We can help clarify which professionals to contact for your situation.
The next step can be a conversation
You don’t have to make
the next decision alone.
Bring your questions. Your uncertainty. Your hopes for what comes next.
We’ll start right there.
